Background
Response to lithium in patients with bipolar disorder is associated with clinical and transdiagnostic genetic factors. The predictive combination of these variables might help clinicians better predict which patients will respond to lithium treatment.
AimsTo use a combination of transdiagnostic genetic and clinical factors to predict lithium response in patients with bipolar disorder.
MethodThis study utilised genetic and clinical data (n = 1034) collected as part of the International Consortium on Lithium Genetics (ConLi+Gen) project. Polygenic risk scores (PRS) were computed for schizophrenia and major depressive disorder, and then combined with clinical variables using a cross-validated machine-learning regression approach. Unimodal, multimodal and genetically stratified models were trained and validated using ridge, elastic net and random forest regression on 692 patients with bipolar disorder from ten study sites using leave-site-out cross-validation. All models were then tested on an independent test set of 342 patients. The best performing models were then tested in a classification framework.
ResultsThe best performing linear model explained 5.1% (P = 0.0001) of variance in lithium response and was composed of clinical variables, PRS variables and interaction terms between them. The best performing non-linear model used only clinical variables and explained 8.1% (P = 0.0001) of variance in lithium response. A priori genomic stratification improved non-linear model performance to 13.7% (P = 0.0001) and improved the binary classification of lithium response. This model stratified patients based on their meta-polygenic loadings for major depressive disorder and schizophrenia and was then trained using clinical data.
ConclusionsUsing PRS to first stratify patients genetically and then train machine-learning models with clinical predictors led to large improvements in lithium response prediction. When used with other PRS and biological markers in the future this approach may help inform which patients are most likely to respond to lithium treatment.

Background
Studying phenotypic and genetic characteristics of age at onset (AAO) and polarity at onset (PAO) in bipolar disorder can provide new insights into disease pathology and facilitate the development of screening tools.
AimsTo examine the genetic architecture of AAO and PAO and their association with bipolar disorder disease characteristics.
MethodGenome-wide association studies (GWASs) and polygenic score (PGS) analyses of AAO (n = 12 977) and PAO (n = 6773) were conducted in patients with bipolar disorder from 34 cohorts and a replication sample (n = 2237). The association of onset with disease characteristics was investigated in two of these cohorts.
ResultsEarlier AAO was associated with a higher probability of psychotic symptoms, suicidality, lower educational attainment, not living together and fewer episodes. Depressive onset correlated with suicidality and manic onset correlated with delusions and manic episodes. Systematic differences in AAO between cohorts and continents of origin were observed. This was also reflected in single-nucleotide variant-based heritability estimates, with higher heritabilities for stricter onset definitions. Increased PGS for autism spectrum disorder (β = −0.34 years, s.e. = 0.08), major depression (β = −0.34 years, s.e. = 0.08), schizophrenia (β = −0.39 years, s.e. = 0.08), and educational attainment (β = −0.31 years, s.e. = 0.08) were associated with an earlier AAO. The AAO GWAS identified one significant locus, but this finding did not replicate. Neither GWAS nor PGS analyses yielded significant associations with PAO.
ConclusionsAAO and PAO are associated with indicators of bipolar disorder severity. Individuals with an earlier onset show an increased polygenic liability for a broad spectrum of psychiatric traits. Systematic differences in AAO across cohorts, continents and phenotype definitions introduce significant heterogeneity, affecting analyses.
Solar extreme ultraviolet (EUV) flare observations and findings from the Solar Dynamics Observatory (SDO) EUV Variability Experiment (EVE)

New solar soft X-ray (SXR) and extreme ultraviolet (EUV) irradiance observations from NASA Solar Dynamics Observatory (SDO) EUV Variability Experiment (EVE) provide full coverage from 0.1 to 106 nm and continuously at a cadence of 10 seconds for spectra at 0.1 nm resolution. These observations during flares can usually be decomposed into four distinct characteristics: impulsive phase, gradual phase, coronal dimming, and EUV late phase. Over 6000 flares have been observed during the SDO mission; some flares show all four phases, and some only show the gradual phase. The focus is on the newer results about the EUV late phase and coronal dimming and its relationship to coronal mass ejections (CMEs). These EVE flare measurements are based on observing the sun-as-a-star, so these results could exemplify stellar flares. Of particular interest is that new coronal dimming measurements of stars could be used to estimate mass and velocity of stellar CMEs.

In perhaps 25 years of creative productivity (ca. 1180-ca. 1205), Hartmann von Aue authored a dispute about love between the body and the heart, Die Klage, numerous songs of courtly love, crusading songs, and most likely took part in a Crusade himself. He composed the first German Arthurian romance, Erec, based on Chrétien's like-named work, and he -- apparently -- ended his literarycareer with a second, Iwein. Further, he is the creator of two provocative rel-igious-didactic works, Gregorius, a tale of double incest, repentance, and redemption, and Der arme Heinrich, the account of a seemingly perfect nobleman who is stricken with leprosy and then ultimately cured by a process set into motion by a very young peasant girl, whom he ultimately marries. Noother medieval German poet treats such an extraordinary breadth of themes at such a high level of artistic expression. The essays in this volume, written by scholars from North America and Europe, offer insight into many aspects of Hartmann's oeuvre, including the medieval and modern visual and literary reception of his works. The volume also offers considerations of Hartmann and Chrétien;Hartmann's putative theological background and the influence of the Bible on his tales; the reflection of his medical knowledge in Der arme Heinrich and Iwein; and a complete survey of his lyric production. Newer avenues of research are also presented, with essays on issues of gender and on the role of pain as a constitutive part of the courtly experience. It is hoped that this volume will prove to be a stimulating companion not only for those familiar with Hartmann but also for those who are just making the acquaintance of one of the greatest of medieval German poets.

The “No-Patient” Run: 2,698 Patients Evaluated but Not Transported by Paramedics

Patients evaluated by paramedics but not transported by ambulance to the hospital (“no-patient” runs or NPR) form a large part of the volume of ambulance runs in many emergency medical service (EMS) systems and account for 50–90 % of litigation brought against paramedics. Since there are no published studies of this important population, this paper provides a demographic description of the encounters and the prehospital patient disposition in one EMS system. Of all EMS runs for 1987 in the system studied, 2,698 (26.1 %) met NPR criteria. Mean run time for the NPR was 18.5±10.5 min versus 43.1 min for patients transported. The proportion of NPR was unrelated to sex or EMS response type, but was increased in those over age 40 years (p<.001) (Chi square), but was less for those over age 50 years (p<.001) compared to all ages. The proportion of NPR was associated with hour of the day (p<.01), with the highest proportion seen between 0000–0559 hours when overall run volume was lowest. The most frequent prehospital assessments in NPR were minor trauma (25.3 % of NPR), blunt head trauma (18.5%), and no illness or injury (15.9%). Most NPR patients were evaluated by medics and released (46.5%), while 22.7% sought medical care by private vehicle. Treatment or monitoring was performed in 17.6% and refused in 23.8% of NPR cases. Patient disposition was: released to friends/relatives (39.1%); presumed alone (35.5%); to police (18.2%); or other medical care (7.3 %). Medical control physician contact was not required, but was used in 2.1% of NPR cases.
This is the first study of NPR in an EMS system. It describes patient demographics, disposition, and differences from transported patients. Emergency Medical Services providers specifically should examine this population in their systems to maintain patient care standards and decrease medicolegal risks.
